Disclosure Statement for Small Business

1. Select Bankruptcy.
2. Select Plan.
3. Ensure correct case name and number match the document you are filing.
4. Select Disclosure Statement for Small Business.
5. Check the box if filing jointly with another attorney.
6. Select the party Filer. If  party filer not listed, Add/create new party.
7. Browse, verify and attach the document (pdf file).
8. Attach supporting documents, if applicable.
9. Modify final text, if applicable, such as:
   • Title of document and/or
   • Pertinent information.
10. Warning!! Verify filing entry is correct before submitting the "Final Docket Text."
