Certificate of Credit Counseling**
  1. Select Bankruptcy.  
  2. Select Other.
  3. Ensure correct case name and number match the document you are filing.
  4. Select: Certificate of Credit Counseling.
  5. Check the box if filing jointly with another attorney.
  6. Select the Debtor as the party filer.
  7. Browse verify and attach the document.
  8. Warning!! Verify filing information is correct before submitting the "Final Docket Text."

**This document is required at time of filing for individual debtors for all chapters unless an extension is requested. This document is supplied by an Approved Consumer Credit Agency. The Clerk's office will send a deficiency notice with a deadline of fifteen (15) days if document is not received upon case opening.
